In the state of Florida, if you are under 18 years of age, you must have a notarized consent form, it doesn't matter if you're coming with your parent or bringing your child with you, this form is still required and there is no way around it. Do not read this and think this won't apply to you. I have too many moms or dads who show up even after reading this thinking they don't need it and can somehow bypass this, you'll only get turned away.
